Anthony Terry DiCostanzo, age 71, passed away on March 10, 2026. Anthony “Tony” was born on May 15, 1954 in Chicago, IL to his parents Joseph and Theresa DiCostanzo. He was born the fourth child after his siblings Joseph, Phyllis and Gloria. Tony was preceded in death by his parents and his siblings: Phyllis and Joseph; and he is survived by his sister Gloria (Bill) Nash.
Tony attended Holy Cross High School in River Grove, IL followed by DePaul University and then the Loyola School of Dentistry where he graduated in 1980 and affectionately became known as Doctor D. Dr. D opened his first dental practice in River Forest, IL followed by his practice in Munster, IN. He later moved his practice to Dyer, IN under the name “Smiles by Doctor D”. Tony was married to Mary Lou (Marten) DiCostanzo in 1979. He is survived by ex-wife and his 4 children: Michelle (Jodi) DiCostanzo, Chrissy (Steven) Brown, Jennifer (Evan) Raber, and Tony (Kara) DiCostanzo; and his grandchildren Tyler Brown, Parker Brown, Grace Brown, Peyton Raber, Regan Raber, and Anthony “Joe” DiCostanzo. He is also survived by his girlfriend Joyce Krukowski.
Dr. D was a fixture among the community of Northwest Indiana. In addition to being a Dentist, he was an avid sports fan and bocce ball player. Dr. D spent many years coaching his kid’s sports teams, and more recently, “Pa” cheered from the stands as a spectator at his grandkids’ sporting events. Dr. D was known by many and loved by all. He was larger than life and will truly be missed.
A visitation will be held on Friday, March 20, 2026 at Westminster Presbyterian Church at 855 Columbia Avenue, Munster, IN 46321 from 4:30-6:30pm followed by a service at 6:30pm. A Celebration of Life will also be held on Saturday, March 21, 2026 at Villa Cesare at 900 Eagle Ridge Drive, Schererville, IN 46375 from 6-9pm. Please wear attire from one of Tony/Dr. D’s favorite sports teams to honor him at the Celebration of Life. Below is a link to a kudos board where friends and family can share memories and photos:
